- Catawba Sports Medicine Physical Form
All incoming freshmen and transfer student-athletes are required to obtain a collegiate sport physical which MUST BE completed on the Catawba Sports Medicine Physical form. Physicals MUST BE dated NO EARLIER than June 1, 2022. Physicals dated prior to June 1 WILL NOT be accepted. Note: A returning student-athlete will be notified if a new physical is required.
- Returning Athlete Physical 2022-2023
Returning student-athletes only, if you did not complete a returning physical in April of 2022, you will be REQUIRED to get a physical before cleared for participated in August 2022 at your own expense. Please complete the physical on the this form: Catawba College Sports Medicine Physical Form.
- ADD/ADHD Prescription Medication Use; NCAA MANDATORY Requirements
The NCAA has strict guidelines regarding medications for the treatment of ADD/ADHD; they are considered performance enhancers and BANNED unless proper exemption paperwork is maintained. If you currently take a prescribed medication, select the appropriate link below for further instruction.

If you are brand new to Catawba College, please select this link for specific instructions as to what is required when you first come to campus; this is MANDATORY.
- Prescription Refill Mandatory Requirements
If you have already submitted the information in the “New to Catawba” ADD/ADHD instructions, please select this link for specific instructions as to what is required for EVERY DOCTOR APPOINTMENT you obtain an ADD/ADHD medication prescription refill as long as you remain a student-athlete; this is MANDATORY.
